I think the EB110 was quad-turbo, too.

I'm not aware of any other production cars.

I suppose it's rare because in the past cars have tended to either have big engines or turbochargers but not both. The most likely application for quad-turbos would be a sequential setup on each side of a twin-bank (V or flat) engine and there historically haven't been a huge number of turbocharged engines with two banks.
