Early W221 - bulletproof or basket case?

I have just over six months left with a leased W212 and I'm thinking about replacing it with something for cash.
Early W221s have the right combination of comfort, modern looks and a standard autobox and are starting to appear at temptingly low prices.
I realise that I may end up looking like a drug dealer, and/or end up buying an ex-taxi with eleventy billion miles on the clock, but let's assume I'm OK with that.
What tends to go wrong, and what tends to be outrageously expensive when it does?
Any recent experiences?
