Application form help

Hopefully a quick question,

I'm currently filling in an application, its asked for the name and address of my previous school. The name of the school has changed recently (since I left), should I put it as the current name, or the name when I was there? I've currently got the current name, but thought I would check just in case.

append "formerly <old school name>" if there's room, otherwise I'd use the current name and that's what they'll answer the phone with if anyone calls to take up references.

I doubt that it matters very much in either case. You can't be expected to keep track of the changing names of everywhere you've been educated or worked.
