Over the past couple of months the AGP fan on my motherboard has been getting louder and sounding more arthritic.

M/B - Gigabyte 8SQ800

question is is it possible to swap this fan out or would it be easier just to install new M/B?

Needs to have USB2 + Firewire.

AGP fan? You must mean Northbridge fan right? Anyway you can get giant heatsinks that don't need fans to replace that with. I've got one on my motherboard. It works fine, no overheating and no noise.

For example this one

Blow all the dust and fluff off it.

You can buy cans of air called airdusters, or just give it a really good BJ yourself.

Seriously!

Once they're noisy that's it usually. I've never made one quiet again by cleaning with an air duster, and believe me, I've seen 100's of 'em.

As linked to above, passive heatsink is the way forward.

Or if it really is the fan on your AGP card, go for one of these

You can use it without the fan as well.
