My R8 has got an mot looming, but since the last one I've had headers, Walky sport cats and Walky 'baby' cam installed - any likely issues with emissions?

Should be fine so long as you get the cats really hot. Suggest that you give the car a good drive first to warm them right up properly, and then ask the tester to test the emissions first while the cats are hot for best chance to pass.

Or just take to a 'cat' friendly MOT tester who is pragmatic about these things
