get peoples browser to refresh when the come back to your site, or should there browser do it automatically?

You can use some meta-tags to put in expiry times for cache, but If they are being forced to use an ISPs Transparent proxy, its down to the ISP anyway.

Most browsers should update at least one time per 'session' but you can override this locally by saying never update.

Ctrl-F5 should force a Refresh anyway... but as before,NTL and the like can put the kybosh on this.

There's also a content expiry date in IIS. Users can override this by arsing about with their IE config without having a clue what they're doing.
