Simple Introducers Agreement

Looking for a simple introducers agreement so we can pay a fee to a company that pass us work. The work is property surveys with a value of a few hundred pounds only, just in case this makes a difference.

After a simple document, is there anywhere someone can recommend getting a draft copy to personalise or is it always better to have a professional draw up a specific document for us. I can't think of anything, clauses etc that would be different to the norm.
