QR skewers coming loose

On a couple of recent rides I have noticed my QR skewers coming loose, both the front and rear on separate rides. Is this likely to be down to user error? Or work out cheap skewers?
I've done about 2,500km on the bike and had the wheels off plenty of times, so it seemed strange that both failed recently. Is it time to buy some bling Hope skewers?

There's a big recall with QR skewers, lots of different brands are affected, perhaps yours are in the same group?

Got one of these on the rear of my MTB. Prefer them to standard skewers.

Never had one come loose though - any sort of skewer on any of my bikes - road or mtb.

Yes, I've been extra careful after the first time I noticed one was loose.Bumping this as I am finally getting round to ordering some skewers. Would these be the correct ones? I'm a bit confused by the sizes, I've measured my dropouts to be 100mm front and 135mm rear, which I believe is standard. However these seem to be listed by skewer length, rather than dropout distance.
