phone calls to USA from UK - dial 00 + 1 then the city code.

If you are only going to use the mobile in the UK either + country code or 00 country code will work - if you are going to use the phone in other countries the only way it will work is + country code, this will also apply to all your UK numbers called from outside the UK.
If you travel regularly it is best to always enter + country code for all your numbers.

My mobile phone worked dialing back to the UK without using the country code from Canada last summer. Wonder if it was because my phone is an office extension rather than an ordinary mobi? ie if I'm dialing the office I just use a short extension code rather than the full number, same with dialing a colleague.

Erm not true, I travel regularly and 0044 works fine.
