How do I set up a survey on PH?

I would like to set up a survey to ask people on here whether they have ever had near misses ie almost had an accident but either they or other road user managed to avoid it (by braking, swerving, saving the unexpected skid, oversteer etc). Ideally would want to ask if people had had either a) none, b ) 1, c) 1-5 or d) more than 5

I think I would fall into the d bucket as have had to take evasive action a few times and also had the odd moment myself.
