BMW F10 520d Hidden fault codes, Jerky 1st gear

Can anyone help with the fault codes in the hidden menu, I have 2:
E12C01
E12C03
I also have a problem when selecting 1st (D) it sometimes jerks violently, and it sometime jerks when it gears down into 1st when coming to a stop.
There is no pattern as to when this will happen I.e. hot, cold or been stood for a while, I just know when its going to happen as when put into drive it does nothing for a couple seconds then jerks rather than feeling the drive engage.
Any help is welcome, if anymore info is required let me know.
