The bore of the hose should be the same as the OD of the pipe it is going to fit over. If it's pressurised you would be advised to provide a bead on the end of the pipe. (Options to do that would depend on the material.)
Various automotive aftermarket suppliers sell fittings to do this type of thing and you may be able to find a fitting with a compression joint to fit the pipe at one end, and a hose barb or standard joint fitting at the other.

Should be possible but how easy would depend how thick it was. If it's steel you also have the option of adding a weld bead. But the bead is only to stop the hose clip from sliding off and you shouldn't rely on that to seal - pulling a 16mm hose down onto a 15mm pipe is unlikely to seal reliably and you should use a pipe/hose combination that fit properly.
