Looking for help (selling car / swapping private plates)

Apologies if this is in the wrong place - I couldn’t see where this would go such as an ask for help section! Please move to the appropriate place if needed.
I’m looking for some help before picking up a lease car in two weeks time. Mu currently Car has a private plate on it which I’ll be putting onto my lease car once I have it. What’s the process for changing my number plate and do I need to send in the V5? I’ll need to get new plates made up for the original reg.... does completing the forms only trigger a new V5 document that I’ll need to show at a garage to have plates made? I’m assuming you can’t just get plates made up without having evidence that it’s genuine to the Car.
If someone could explain how this process works it would be really helpful.
