I haven't seen much about this incident, but it wouldn't surprise me if there was some element of 'GPS assisted collision' involved. When entering way-points either from a paper or e-chart it is all too easy (and human nature) to choose an 'object' as the WP...be it a buoy, an intersection of lat and long, even a sounding.
Only takes two vessels to have chosen the same WP, and in a busy shipping area (which I think I heard this is), then it is only a matter of time before you have a potential coming together. I don't know if any other these applied, but factor in weather, visibility, day/night time, crew experience, crew attitude/diligence, equipment (radar) failure/mis-calibration and suddenly the 'potential' becomes much closer to an actual coming together.
