I've had a sit down and a serious conversation with someone I know that has been recovering from major brain surgery (epilepsy).

It was a stormy recovery for the first few weeks, REALLY struggled, but in time things begun to settle down.
He's been pretty closed off with the stresses off life since the surgery, he's not had too many dealings with the outside (the surgery took place in March last year).

As he slowly began integrating into the outside world, with work etc, we began seeing a personality change. I really wanted to get inside his head, try figure out what's going on. A common side-effect post surgery was anxiety and depression, he's definitely had a few breakdowns that have led to now having therapy for anxiety.

More worrying have been his mood swings, not so noticeable from the outside, but speaking to him, it does sound pretty intense. Having the ability to tame himself back from doing anything he regrets is keeping things in check, but talking about (for example) something/someone done to him (being picked on by a manager at work) that's annoyed him, and what goes through his head to get his own back are pretty DRASTIC. I'm not really sure where to go from here, he looks fine (apart from the huge scare on his head), he generally acts ok, but he sounds mentally unstable when sitting down with him, and he's opened up to me).

The NHS are all over the place, I'm not even sure he'd open up to a GP, or his consultant if he was to see them, but there's no appointments in the pipeline to see his consultant/neurosurgeon anyway.

How do I get some professional advice on the matter?, what to do, where to go?
