Is it a medical problem? Can't function when tired or hungry

This is a topic that must have been thought by every bloke ever. My wife is completely useless when hungry or tired.. I'd liken it to a toddler - can't respond coherently, illogical, irritable..
I get that it's a normal reaction to hunger or tiredness but for me to reach the same level I have to not eat or sleep for 24 hours or more - for her you are talking 5 to 6 hours to reach the same level.
Due to unrelated stuff she's had recent blood tests with no abnormalities.
She has a very fast metabolism generally, is this just normal genetic variation or do people have stories of it being something more?
