Recommended personal alarm for elderly parents

My father is 91 and my mother is 86. They live 3 miles from my house and my sister lives 100 miles away. Until 3 months ago they have lived amazingly independent lives. They both still drive and they are involved in many social activities (bridge, church, my father swims etc). My mother was diagnosed in April as having Heart Failure. This is a degenerative condition although she is not likely to be in immediate danger.
Both my parents are suffering from slightly reduced mobility. We have a good medical and care structure in place and my wife and I can keep in regular contact.
What I would like is a really good alarm system for them both. Ideally to work at home and also if they are out. A panic button that would alert me and my sister if either of them were in difficulty.
It doesn't have to be a monitored system but I am not ruling that out either. Cost is not the primary consideration.
Any recommendations based on other people's practical experiences.
