Fiat Punto diesel intermittent

I've got a 2012 fiat punto 1.3 diesel. It's a great car, pulls well, very economical. Theres a problem I've had with it the whole time I've owned it and I've never been able to fix it because it happens only occasionally.
It tends to be after going for 3 or more miles at the same speed, around 70. First thing to note is stop start doesn't stop the engine when I come to a halt. The idle bounces between 600 and 1000 when it normally sits at 800 to 850. Then theres a delay in throttle response when i come to set off, and if times with the revs being at a low point, I come very close to stalling. Naturally I then floor it because it's not doing anything. When it does pick up, it sends a reasonably sized plume of smoke out the back. I'd say its black smoke but quite thinly spread so just looks grey. If I then lift off the throttle and push it down hard again, boost has completely gone and takes a couple of seconds to regain power, no matter the revs. Same goes for gear changes, which is frustrating and quite disorienting. No engine codes when it happens, and it tends to last about 5 minutes of driving before going back to normal, switching off when it's like this and waiting 10 minutes or so also sorts it, though just briefly switching it off doesn't.
It's not losing any fluids at a rapid pace and as I say, it happens fairly rarely, maybe twice a week doing about 300 miles a week. I thought perhaps it could be a dpf cleaning cycle but I dont understand why it would be so intrusive to driving. Any ideas?
