Can anyone identify this cane/topper

Bit of a random one this but there's a big audience on here

I found this cane and topper the other day in the attic



Its a 3 piece cane and the topper is a brass pump, there is a piston in the back of the brass box behind the wheel that moves up and down when the wheel rotates



The support arms for the pump are unfortunately broken but I'm intrigued as to what it is, purely decorative or has a use (medical maybe?)

Also is it worth anything!
