Cigarette lighter only giving 0.4v??!

You are going to have to get a multi-meter out & test back if the fuse is OK but it is basically getting nothing.
You say the fuse is OK, but is there 12v at the fuse?
You say you are getting basically nothing at the lighter, how are you actually testing it?
Have you checked the fuse since testing it, as it is really easy to short them out when testing?
If there is 12v at the fuse, the fuse is OK but there is basically nothing at the lighter, there is either a high resistance joint in between or a cable break.

Car was running when testing with a tester.
Its not getting enough power for some reason

Need to check volts at the fuse box then.
The wires are just for illumination.
I have a 2008 mondeo mk4
3 spade connection on car
Connected it to the spades as normal and grounded black wire to ground on spade connection.
The issue is on the car wiring to the fuse box though, those cables (that plug into the cigarette lighter spades aren't getting enough juice)

I will try the light thing but dont think its that.
Was working for a bit when 1st had the car, all i have done is disconnect the 3 spade connectors from the lighter.
Dash illumination works fine.

Would it be anything to do with the alternator? Noticed my interior light flickering yesterday.
If so.. anyone know how to test a "smart alternator" (2008 mondeo)

You should have one wire close to the vehicle ground voltage and the other close to battery positive voltage. If you don't get that, your actual voltage readings will guide you to the problem.
