Retired CMIOSH here. I spent 30 years trying to get people like your boss to work safely.
So many failings in evident from your narrative!
Phone HSE and a make a report asap. They will log it and await your employer's report.
Your employer will be in even deeper poo if he fails to report this as a RIDDOR LTA at the earliest opportunity.

Hope you recover quickly and without complications down the line, and don't lose your job over it, but...
Consider yourself lucky for not killing yourself or somebody else by not reporting it before now.
In my eyes, you are equally as responsible as the owner for not reporting it, and then carrying on using unsafe equipment and tools you're not trained/licensed for.

Without a doubt report as others have said, that breaches so much. Missus is an ex HSE inspector. The only time she lost it (this was when she was in her 20s so about 30yrs ago) was responding to an accident where a young lad new to a job doing almost exactly what you've been doing had a fork lift topple from a ramp it shouldn't have been going up in the first place onto a wagon onto him. He was still being removed from the scene when she got there as she just happened to be near and was making a lot of noise. He ended up a double amputee but lived.
In "losing it" she basically bounced the company owner round the site office such was her anger. Her early twenties 5'5", size 6. Him 6ft and about 16st.

Lawyer here who looks after H&S for large national business:
1. Report to the HSE ASAP.
2. Get legal advice ASAP - personal injury firm is fine.
3. It is compulsory for him to have employer's liability insurance - tell him you want to make a claim and need the details ASAP.

Thanks. As soon as I’m out of Hopsital I will call HSE. Just awaiting theatre now. So many failings in evident from your narrative!

Didn’t go straight to hospital he sent another lad out that has no HGV license to drive the truck back with me in it, then I got a lift to the hospital

Make some detailed notes of the events both pre and post incident before your memory fades. Contemporaneous notes are worth their weight in gold.

Regarding compensation which is duly deserved and the no doubt additional costs/lost work you're going to experience I would instruct a solicitor ASAP.
With regards to the company potentially folding, I'm not sure how this would impact you or if it will, providing he is insured and has employee liability which he is legally obliged to do. Obtain the details of this if possible, if you don't want to ask him it should be displayed in a prominent position on the premises, perhaps a trusted friend could help.

This as well.Doesn't have to be written in legal speak.
Just write down what happened. Bullet points help as a timeline:
- On Tuesday 13th of April at about 10am Mr ABC asked me to do XYZ
- On the journey from A to B the brakes were failing, smoking and the pedal went to the floor on XYZ road
- I told Mr ABC at the end of the day and... etc
