Vauxhall Insignia EGR excessive flow code

Hi all. Got a 14 plate Vauxhall Insignia 2.0 TDI that is going into limp mode on pretty much every journey. OBD2 always comes back with the following code:

p0402 - Exhaust Gas Recirculation Excessive Flow

Car has had a new EGR valve and also a new mass airflow sensor to try to resolve the issue. We also found a perished pipe that was replaced, but both my mechanic and the main dealer seem to be out of ideas as to what to try next.

I'd be grateful for any suggestions or advice on known common issues that could be causing this, or what could be tried next.

I believe a new Egr valve sometimes needs to be adapted when fitted no not sure if this was carried out?

@Belle247: That definitely sounds like something worth checking. Will get in touch with the garage that fitted it and make sure. Thanks for the suggestion!
