Mallory Entry Form

Would someone be kind enough to fax me over a blank Entry Form for the Mallory race in September, tried to get one off BARC but I go through the press 1 for this, press 2 for that, press 3 for...ARGH, JUST LET ME TALK TO SOMEONE!!!

...Anyway spoken to BARC a few of times and they've promised to fax one over but nothing has arrived, could someone here oblige?
