Foxwell NT530 O2 sensor mapping

Hello,
Has anyone mapped out or does anyone have the mapping of the individual senors to the Foxwell "EMS Cylinder 12 (bank 1)" and "EMS Cylinder 12 (bank 2)" fault codes?
I am starting to get a repeat on "EMS Cylinder 12 (bank 2)", code O2 sensor Bank 2, Sensor 1.
If i was to guess, my guess would be Cyl 10-11-12 Upstream

Here's an example i what I'm trying to get together
EMS bank 1 “Code bank 1 sensor 1” Cyl 1-2-3 Upstream
EMS bank 1 “Code bank 1 sensor 2” Cyl 1-2-3 Downstream
EMS bank 1 “Code bank 2 sensor 1” Cyl 4-5-6 Upstream
EMS bank 1 “Code bank 2 sensor 2” Cyl 4-5-6 Downstream
etc...
