Eugh that's rubbish, and makes it even worst if possible : ( It certainly looked like there are going to be more casualties on the ground as well - some terrible footage of someone walking out of the area completely engulfed in flames (don't go looking for it).

I’m not saying this is the case in this tragedy, I’m just speculating. There have been a number of Medevac incidents over the last 12-18 months in the USA. Observers on YouTube (Juan) have commented they often operate with just one pilot.
I remember one incident from about 12 months ago where the pilot lost spacial orientation at a high altitude - and that too descended like a dart.
