Blood pressure monitor - remote monitoring

I'm looking for a blood pressure monitor that will effectively upload results automatically (i.e. hopefully with no extra steps) so that my elderly relative's results can be viewed by me.
They have a modern Samsung mobile phone.
Does anyone have any recommendations please?
Ideally it would be as simple and seamless as possible once set up.
