Youngster with hair loss

One of my sons is 19 and already losing his hair. His grandfather on his mother's side was balding in his teens too.
The doctor has suggested "Regain extra strength hair loss foam" and also Finasteride, which he would not be able to get on the NHS.
The lad does not have a high T look about him. Weak facial hair growth, struggles to gain muscle.
My thinking as a layperson is that, if he already isn't very "testosteroney" that taking finasteride may not be the right way to go.
He is thinking that he wants to slow his hair loss until his mid twenties, when he will look at a transplant.
Anyone any experience of this subject please? Any thoughts?
Not much to add being as you've already hit on T-levels, but I was having a discussion with my brother in law who's a senior nurse earlier today and he was saying elevated T levels thanks to the gym bro culture is causing a lot of premature hair loss.
I'd suggest, as I'm sure you probably already have in mind, is not letting it knock his confidence. Get him to have a chat with his GP and let him know that some chaps just lose the lot at an early age.

High testosterone doesn't necessarily mean high DHT and baldness. And low T doesn't mean you won't go bald. It's more about genetics and hereditary than T levels.
I'd definitely steer clear of finasteride but minox by itself might do something. Any intervention takes months for any noticeable difference to show up though. Take note of the instructions with minox i.e. you could see an increased shedding phase when you first start using it, before a new growth phase comes along.
How's his diet and nutrition? Certain supplements and vitamins could provide a little help.

I had a mate who all through school had an awful hairline, we'd take the piss out of him something rotten. One day he told me he'd shaved it all off, mid-20s. Couldn't wait to see him to take the piss (horrible, I know). Turns out it was the best decision he could have made, looked miles better and there was no piss to be taken.
Maybe he just needs to take the plunge, could be liberating. Wish I had the balls to do it sometimes.

Not true. Minox by itself will not stop hair loss. Finasteride is the only thing that will. Minox is good to use alongside Finasteride as it encourages new hair to grow.....but 100% his son needs to take Finasteride to slow / stop the hair loss. What specifically isn't true?Although I'll chip in with something that isn't true, in that the side effects of finasteride are over blown. That isn't true. A 19 year old does not want to be taking that for the rest of their hairstyled life.
Finasteride may increase T levels because less will be converted to DHT. However, more T may then be converted to estrogen which is partially responsible for the unwanted sexual side effects.
Old 'low T' forums were full of men who'd found themselves on TRT because they took finasteride and things didn't go back to normal when they stopped. Not worth the risk in my opinion, not for a young person.

The only thing is a hair transplant, if it bothers you that much, things have moved on in the last ten years or so, and some of them are very very good, Not cheap, but only the cost of a nice Cruise.

If it's at that stage, just support him in cutting it short. Everyone will quickly get used to it and he can stop worrying about it for the rest of his life.
The other path is going to be expensive, have concerning side effects and at best only postpone the inevitable while he worries about it daily for years and years.
