Check its status in disk manager and check the driver details.

If you stop the explorer process (and hopefully it will restart itself) does it disappear?

I *think* it happens when the driver database throws a hissy fit.

Happens occasionally, only seen it a tiny number of times.

That could explain it; I *have* been trying to get an appropriate driver for my phone installed (that's another issue entirely!)
