Last weekend I dutifully checked tyres, topped up windscreen washer, oil, etc before going out for a spirited jaunt up to Wotton, Tetbury, Cirencester et al. Stopped off for lunch to see smoke gently wafting from my bonnet. 
I'd forgotten to return the oil filler cap! It had fallen onto the exhaust manifold. Lots of smoke and melted orange plastic. Fortunately, very little oil 'escaped' and the cap had not distorted so once it had cooled down I was able to fit it.

I did that but with the Leven Filler cap. I left it on the flat bit of the engine conveniently fitted in front of the filler, and said filler remained there for a fair while, finally falling off and bouncing into the grass at the side of the road. Luckily (we were in France at the time), I found it almost straight away. I too noticed how little oil had been vented, given that we'd done at least thirty miles before the cap fell off.
That makes two of us in this little club, LG.
I did a similar thing long ago when looking at a Vauxhall Nova I was thinking of buying. I took the oil filler cap off to check for 'mayo', failed to replace it correctly, went for a test drive and on return opened the bonnet to check that there were no unusual rattles only to find copious amount of oil spread over the engine and engine bay
