Which brake fluid should I be using in my '99 4.5?

Dot 4 is the recommended one, and will do the job, but 5.1 if you love the car.

Would the system have to be flushed if it had 4 in already?

most definately..DO NOT MIX mineral (dot 4) and synthetic (dot 5) fluids.

DOT4 and DOT5.1 are freely interchangeable and can be mixed, DOT5.1 is a slightly higher standard and is preferred.

DOT5 (without the ".1" ) is a completely different beast, shouldn't be mixed with the other types and is not usually considered as suitable for road use.

Dot 4 ok for road use,you dont need anything more than that unless you intend to do some serious trackdays.
