Voting Dividends

Hello chaps.

Can anyone point me in the direction of a template for deciding upon and voting dividends that I could complete for my company and hold on record?

"It was decided at a meeting of the directors on (date) that a dividend of "X" pence per share would be paid in respect of the profits for the year ended (or period ended) DD/MM/20YY".

The above would constitute the minuting of the decision by the directors to pay the dividend.

Separate dividend vouchers for each shareholder showing the details of the dividend (Net Dividend Payable, Related Tax Credit, Dividend expressed as a fraction per share, name and address of Shareholder) should also be produced for pasing on to the shareholders.
They will need this information and paperwork when completing their personal Self Assessment tax returns.
