Mondeo 1.8 engine problem

1999 (Mk. II) Mondeo 1.8 petrol manual, around 84,000 miles.
When cold, at fast idle, the idle isn't even. It hunts with an even variation of about 50 RPM: revs go up for about a second, then down for about a second then up again, in an even pattern. At normal engine temperature the idle is rock steady.
Under hard acceleration there's a dip in power, doesn't sound like a misfire and doesn't always do it, but nearly always. Finally, at speed eg. uphill on a motorway it can run out of power. Change down to fourth gear (and therefore higher revs), power returns but there's quite a lot of induction boom (higher revs, throttle wide open).
For info, car has always been serviced regularly by main dealers, last one a major service about 2,000 or 3,000 miles ago. Problem has existed since I bought the car about 5,000 miles/6 months ago but seems to have worsened a little, especially as the weather's cooled.
I suspect the two things are related: anyone have any ideas?
