Custom notebooks

What companies would I approach about ordering customised paper notebooks? The sort of notebooks you might use in an office or for college note taking, spiral bound, lined paper - the standard sort of thing.

Don't know at the moment, this is a bit of research I'm doing. Say 500, but potentially a lot more. Hard cover, ~150 ruled pages, spiral bound, first few pages to include some b&w school information (standard sort of stuff - planner, contact info etc). covers (inc insides) to be printed in gloss colour, as well as 5 "dividers" within the notebook (front and back). Any idea of rough cost?

ETA A4 size.
