Online Shopping Carts - Advice Please

[Spam]

Hi,

I'm not too sure about this so was wondering if someone with some experience could advise me.

I have a website at present and wish to offer 3 "packages" for users to purchase.

I also require users to fill out a form first with their specification then select the package they require then add it to the basket and purchase. Ideally this would then send a confirmation email to the user and an email to me confirming someone has ordered a "package"

So to summarise I need:

1. A shopping cart/basket/payment system solution etc.
2. An online form which enables me to personalise the fields I require.

As I say I need advice at this stage on both of the above.

Can anyone recommend a checkout system?

I have notice a few form providers but require a month payment based on the amount of submissions you require to recieve, can I do this for free if I design my own form etc?

N.B When I say packages I don't mean parcels etc.

Thanks,
