There are examples (again in the classic series) when the Master was seemingly dead only to pop up again. I'm thinking specifically of The Sea Devils, when the Master was proclaimed dead only for the Doctor to remove the mask to reveal it was an innocent man, while the Master made a cool escape in a hovercraft.
There is every possibility that the person who died was not the Master, but a hypnotised lookalike/disguised person, which would also explain his "refusal" to regenerate. He "refused" because he couldn't.
