There is an idiot in my office who sits at his desk moaning about how bad his job is, how crap his boss is etc etc. But the best bit about this is that he always loves tellign us that hes going for a great interview here, there and everywhere.He also goes as far as saying "I won't be workign in this team for too long as I'm lining up a job with xxxx".
Amazingly his manager hasn't tired of this and just given him the boot, but more hilariously, he's never actually got any of the jobs he's been telling us about.
As for the original question - definitely sickness/doctor's appointment and do not tell anyone particularly your boss.

Either go with "Need to leave early for embarasing personal reasons" (well it would be an embarrasing situation if you told them it was for a job interview) or just book a day/half day off.

Agreed. If you don't get it no harm done in terms of perceived commitment to job by manager/colleagues and if you get it then you can give the relevant notice etc and still not mess anyone about. If you think you can get away with it tell your employer you are looking for a new job (but dont mention you are interviewing as they get a bit annoyed about that for some reason

If you are any good/valued you may get a pay rise. I guess it depends what field you are working in. I've worked in specialist IT for 20 years and every time I've offered to move along more money has suddenly appeared.
The money never made me change my mind and stay but it was nice to have some extra readies while working out my notice ;-)

That always annoys me. You tell them you're leaving and they offer you more. If they think you are worth the exta money, why aren't they paying you that already? Really gets to me (although, i am in recruitment).
