Last few weeks with all the snow and mud on the ground I have been managing a flat a week. Currently have a slow puncture on the rear awaiting work. Normally I am ok sighting lines to avoid the most sharp objects on a trail but of late starting to get a little tired of it all.
Are tubes with the slime/sludge type of pre loaded sealant a good idea do these work, are there any other options to use on minimising punctures. I tend to keep the tyres inflated high to prevent pinching is this the recommended approach?

It sounds as thought you've either been very unlucky, or your tyres are getting a bit old and worn and just more susceptible to damage (may even be small splits in the tyre causing pinch punctures but only obvious when inflated). The slime filled tubes work, but add weight to the wheel at just about the worst point, which you may not want.

What happens if you get a thorn/something sharp puncture, does the Wheel Milk work like the slime stuff in a tube and seal the hole?Are tubes with the slime/sludge type of pre loaded sealant a good idea do these work, are there any other options to use on minimising punctures. Yes, the wheel milk (or any number of other names for it) seals the puncture with usually minimal deflation. Occasionally it deflates more, but then you just spin the tyre a little and pump it back up to the desired pressure.
Only real failure is if you split the tyre, but then again a tube wouldn't be any help then either.
