But seriously... Yes, they worry me. Not because I think they have much chance of winning any conflict, but because the country is starving and the leadership is desparate. Rats in a corner spring to mind. Under such circumstances, dictatorships often try to distract and unify the population they suppress by inventing threats or even conflicts. Their armed forces (including their arsenal of long range missiles) have the potential to inflict crippling amounts of damage to South Korea, Japan and other regional neighbours.
Then there is the unanswered question of which way China, with the biggest army in the world, would go in the event of such a conflict. Would they support they old ally, North Korea, or would they stand back? Scary.

Ah, 1984 rings true again...... Are they much of a threat? Well, I suppose they are realistically. We could put enough tactical nukes to render the entire country obsolete for 1000 years. BUT, I don't think anyone ever will order that kind of attack anymore. Soo, would we be back to conventional warfare? As mentioned above, the question would fall to which way China goes..... If against North Korea......
