I think my starter is on its way out although I hope it will get me to Chatsworth (and back!) tomorrow.
Everything sounds fine although it seems that the starter cog isn't engaging with the flywheel. It takes two, three or four attempts before it finally turns the engine.
Anyone any ideas what the problem is and how easy is it to get to the stater motor on a '98 4.5?

Sounds like the solenoid is buggered.
You can replace the starter from the top. Take the panel by the wipers off and then remove the fuel rail (but don't disconnect the hoses). You can then get to the two hex bolts that hold the starter on.
You need to give them a good whack to loosen them, then they should come undone easily. Try not to lose the bolts down the hole by the flywheel!!
You can then drag the starter through the gap between the engine and bulkhead....

How much of a good whack are we talking to loosen the hex bolts and what are the torque settings when putting it back together?
Also, anyone know any good re-manufacturers in the Nottingham area?
