Over-revving - Any ideas?

My wifes pre-2006 Cooper automatic has started to randomly rev (even after taking her foot off the accelerator) and is slow to change gear. Putting it into manual mode improves it a bit, but the revs are still random.
It's been back to the dealers twice already with the same problem, once to have a 'component' replaced in the gearbox and once for a 'gearbox oil change'.
It's started again. Whilst I would like to believe it will be 3rd time lucky I get the impression the dealer are just replacing the usual suspects and hoping that solves the problem.
Has anyone come across this issue before and what was the solution?
