There are many, many adjustments that affect fit. To be fiar - yes, you probably will get a bit sore on your first ride.
But if it continues you need to be methodical and measured in your adjustments - saddle height, fore aft, stem length/reach, angle of your riser bars, angle of brake levers, angle of cleats etc all make differences.
As for the cleats - sit on the edge of a table with your legs dangling over - this will show you the 'natural' angle that your feet all at, either in or out. use this as a base to set your cleat angle.
