repairing stud wall nail holes

Have a few holes appearing where the nails are for the stud walls in the house. Most have just slightly blown and havent split the paint yet but I have a few that the plaster has fallen out of leaving me with a 1-2 inch 'hole' i the wall with the nail head visible.
Have tried the usual Polyfila but they end up either coming back or look unsightly. Probably due to operator error.
Anyone give me recommendations as to how to correctly fill and fix once and for all...

First off Kev, re-fix the wall approx an inch away from the nails that have 'popped' using screws, this will sort out the wall, then when you fill them and the existing nail heads the problem shouldn't happen again.
Doh !!! should have thought to ask you first mate.....its a f~*ker as the walls have all been painted a couple of months ago over existing paint, so it looks as if I'll have to redo the whole area. Got some coming through on the ceiling too..What causes them? poor workmanship on install or just building movement?
