Health Questionnaire

I have a new employee starting tomorrow and was wondering if anyone had a Health Questionnaire that they could send me. Have found a miriade of versions via Google Search but most are public sector documents and seem to be quite over the top and invasive, I don't want to scare my employee with paperwork and form filling on Day 1.
