Wow bad luck there. The logic is confusing though. You would have thought anyone inteligent enough realise a tyre change was prudent would also realise they should make the tyre safe by deflating it. I didnt realise a car tyre would be able to kill though, poor woman I guess she didn't either

Holding a car tyre is not normally a hazardous pastime, probably this is unique.Though I suppose you have a wealth of experience of people who have been maimed by car (not truck) tyres?

Surprised at that; I've heard of aircraft and truck tyres blowing and killing folk, but a car tyre? A mate was standing next to a tyre that burst on a boatyard travelhoist(120psi and 5ft in diameter), fortunately the shrapnel only put holes in the shed(two layers of 0.8mm steel with rockwool between) so only a change of pants required and no hospital or ambwilans.
A tragic loss, a freakish occurance.
was the tyre being pumped up at the time? or was it the one with the bulge in the side that she and her husband had just removed and were putting it in the boot?............maybe the tyre was very hot from running and unfortunately went bang at precisely the wrong time!...Unlucky.

Problem caused by "Space saver" tyres.
Stupid bloody idea that the tyre you've taken off doesn't fit where the spare came from.
Not a bad idea in say a range rover but in a sportscar?
I had the same problem years ago when I had an MR2 Mk2.
You have a puncture, take the wheel off, take the spare from under the bonnet, put that on....
Then what? The normal size wheel doesn't fit. Put it on yer passengers knee?
