tax disc refund

Have searched to no avail. What supporting paperwork do I need to take a partially spent tax disc back to the post office and claim a refund please?

They are pretty useless though.
Make sure they pay you EVERYTHING owed. They have twice missed a full month off the refund value when I've sorned vehicles. They put it right when challenged though.
