Bought a car with private plate, how to transfer to PO

I bought a car with a private plate on the understanding it would be transferred back to the previous owner when the V5 came through to me.

Knowing nothing of the private plate process what do I do? / what do I expect?

Can I just fill in a form and and then put the old plates on?

Does a new MOT/tax disc come through to me - do they give a specific "changeover" date?

PO will foot any costs.
