Restaurant fails

Whats the best worst restaurant moment you've ever had? Not just bad food, bad service, we've all had those, but time they've gone above and beyond the calls of atrocity.
I can think of two.
Ilala lodge, Victoria Falls, Zimbabwe. They had a creme brulee as dessert. My wife decided to give it a go, surprised it was back on the menu as it was off where they'd been using powdered milk for the last few days. Seems, they still were - powdered milk creme brulee.
Intercontinental, Lusaka, Zambia. They had a buffet in one of their restaurants which we tried as their main restaurant was rubbish. They had buffet style desserts, including 2 swiss rolls, one strawberry, one chocolate. Now, normally the white crystals on the outside are sugar. Not here, oh no. They were salt. On both.
Anyone else had such impressively bad moments?
