Small Business Account - who is good at the moment?

Looking for suggestions for a small business account provider - some have penalties if you don't sustain a significant balance others have low fees.

Who is good at the moment?

While Lloyds charging structure is quite pleasing (for the online only accounts) the service for a small business has been terrible. I've tried on 3 separate occasions to get a debit card on my account now, the first two I was told the account was "too new" and the third time, after 8 months, they didn't bother to phone me back.

Their online banking system looks like it was developed in the 80's too.

I have been with HSBC for 10+ years. Overall, cant really fault them. They are easy to contact by telephone - and this for me is worth its weight in gold imho.
