T350 Drivers door glass shattered :/

Whilst filling up my drivers door glass shattered . I had shut the door normally and was in the process of topping the petrol tank off when there was a load bang followed by the shattered glass falling into the car.
Is it possible for the window motor to over lift the glass pressing it into the roof and causing it to shatter?
I've located a spare glass from clever trevor and will be fitting it shortly but dont want a repeat incident obviously.
Anyone come across this before? Note prior to the incident the window was working fine and had been for some time.
