In a search of the forum I found only one other post where this happened. My question, does this indicate an underlying problem - other than a couple of loose screws. The car has about 55k on it and runs fine with the new IAC securely in place.
Finally, a major thank you to the Fact File for the cross-reference page. Having the GM part no. for the IAC was a blessing as neither Autozone nor O’Reillys has the Lotus in their database. At least not for this part.

I have had a '91 SE for about two years now, and I have learned that any intake component that is not securely fastened can and sometimes will shake lose due to the boost pressures that these cars produce. In my experience I have had some hoses pop off, but never an IAC valve. These have alan (sp?) screws that should hold them securely. I am a little surprised to hear of your problem...but not too surprised. I wouldn't worry about it in any case. I don't think that it is a systemic problem. Just make sure that you use the proper screws to hold it in place, and maybe incorporate a small lock/star washer for insurance.
